Application for a CRB check
Before you begin
Please make sure that you have the following information before starting the application process:
NB: You should only apply for a CRB disclosure on this site if your company has registered to use our services to obtain checks, in which case your company will have given you a purple ‘Contact Us’ business card with a unique company ID number and telephone/web site details. Individuals cannot apply for a disclosure in their own right (eg the self-employed).
- Company ID Number: You should have a purple CRB Checks business card with the unique ID number of your company.
- Personal details: You will be required to fill in your title, gender, forename, middle name and surname. Note: it is important to record your name exactly as it appears on your passport.
- Your title and position: On the drop down list, select the category that describes your job.
- National Insurance number: If you have a N.I. number, you will need to record this on your form. If you haven't worked in this country you may not have a N.I. number, in which case leave it blank.
- Date of Birth: Record your date of birth.
- Country of Birth: Record your country of birth.
- Town and County of Birth: Record the town and county/state/district where you were born.
- Nationality: Record your nationality.
- Current address: You will need to provide details of your current address and you must be able to prove this with recent documentary evidence (eg- recent bank statement).
- Previous addresses: You will need to provide details of the addresses that you have lived at for the last 5 years.
